A woman in her 30s presented with a 3-day history of fever, coryza, and dry cough. She gave no history of recent travel, drug intake, or relevant exposures. On examination, she had tachycardia, tachypnea, and was febrile with a skin temperature of 104 °F. Bilateral conjunctival congestion was seen. Her buccal mucosa showed multiple discrete 2- to 3-mm pinpoint yellow papules and blue-white spots with peripheral erythematous rings on the right side that were opposite the second molar (Figure). The next day, she developed a pruritic erythematous maculopapular rash over her face and trunk that spread cephalocaudally. The mucosal lesions coalesced, and there was diffuse oral congestion that resolved before the rash. The systemic examination results were unremarkable. A diagnosis of measles was confirmed by the presence of antimeasles IgM antibodies in her serum. The patient later recovered without any complications.
